I have read the packet and understand I am responsible for the following fees: 1. Tuition of $99 for tiny performance and $114 for youth/junior novice. First month's tuition is due on the 1st of the month starting Nov. 1 for youth/junior novice and Jan 1. for tiny performance. 2. Uniform fee of $100 due Jan. 1 for tiny performance. Uniform fee of $175 due Nov. 16 for youth/junior novice. 3. Professional fees of $100 for tiny performance and $167 for youth/junior novice due upon registration.

By signing this, you acknowledge that refunds cannot be granted for unused tuition, uniform fees or professional fees. All fees are promptly paid to the companies on your child's behalf, and our gym is unable to obtain refunds if athletes are sick or cannot compete for any reason.

Your child's participation is extremely important. We understand vacations come up and children get sick causing them to miss practice. Because we only practice once per week for a very limited time, we ask that they not miss for optional school activities, other sports and/or other extracurricular events. Cheerleading is a sport that requires every athlete's attendance in order to be successful with our stunts and formations.

I am aware of the nature of this activity and I hereby assume responsibility for the participant listed above to participate. I acknowledge that, while not common, any activity that involves height and motion (such as bounce houses, tumbling and stunting) involves risk of injury ranging from minor (bruises and sprains) to more serious and catastrophic injuries. I will not hold Central Missouri Twisters (Twister Sports) or its employees responsible in the case of accident or injury as a result of this participation. I understand that this completed form must be in the possession of Twister Sports prior to participation in any program. If, at any point, I have a question or a concern regarding the safety of my child or the intent of the program, I will contact the owner, manager or instructor immediately. I also allow the participants listed to be photographed and videotaped for publicity and advertising purposes only.
