Do you have any scheduling challenges for the upcoming season?*

Please understand that teams are based off the athletes' ages and skill levels. Knowing the day and time of practice in advance is typically not possible. We will attempt to work with schedules as much as possible, but there are many moving parts when scheduling teams and placing athletes, and we will not be able to accommodate everyone.

If you're open to multiple options and want us to select the best fit for your child, please check all that apply. As a reminder, "Novice Only" athletes do not need to try out and can register at the front desk or through this form. Please ensure you've reviewed the packet for time and financial commitments for each of the programs you're selecting.

Novice (Local travel only, 1 practice per week)

Prep (Semi-travel, 1 practice + tumbling per week)

Elite (National-travel, 2 practices per week)

Elite Team Athletes*

1. Athletes on elite teams may be required to attend an off-site practice to participate in the conditioning program. Carpooling and/or the Twister bus will be available to and from Twisters.
2. Elite athletes may not miss practices for other sports practices without prior approval from a coach. We will work with athletes on games, but due to the nature of our sport, Twisters practices and competitions must take priority.
3. Elite athletes who receive bids (typically all teams) will attend the Allstar World Championship in Orlando, FL in April 2027.

Check ANY or ALL that apply. As athletes get older, they often play key roles on teams to help create a more competitive team. Some athletes have level 2 tumbling and level 3 stunting. Placing them on a level 3 team means they may not tumble, but their team can be far more competitive in stunting. The same may apply for those with strong tumbling skills and weaker stunting skills. Please understand we will do our best to accommodate your requests below, but teams are formed first based off the needed positions for stunt groups.

TEAM AGREEMENT

Statement of Understanding*

I, the undersigned hereby certify that I am the parent or legal guardian of the child listed above.

The athlete desires to participate in Twister Sports All Star Cheerleading for the 2026-2027 season, beginning May 12, 2026 with tryouts and ending April 30, 2027. This team agreement sets forth the terms and conditions under which the athlete may participate in the Twister All Star 2026-2027 Competitive Cheerleading Season.

Responsibilities of the Parent/Legal Guardian and Athlete:

Athletes hereby agree to attend all practices, competitions, and mandatory events. I understand and recognize the importance of attending all scheduled practices and team events. I understand that accepting a position at Twister Sports entails a commitment to the gym and fellow team members. While exceptions can be granted for special activities, I understand the importance of having each team member present for all practices. Competitions and performances are mandatory, and I will be responsible for ensuring my participation in all team events.

Attendance: Parents and/or legal guardians hereby agree to the attendance policy as outlined in our athlete handbook. I understand that it is my responsibility to check emails at the contact I provided so I may stay informed about upcoming events and practices.

Finances: I understand tuition is due on the 1st of each month. A late fee is assessed on the 5th of each month. The grace date expires on the 11th, and my child may not participate if any fees are overdue at that time. It it is my responsibility to provide the required uniform items for every competition.

Automatic Debit: I agree to allow Twister Sports to automatically charge the card on file for monthly tuition fees on the 1st of each month if there is not a credit on the account. I also authorize the charge of all professional fees +5% convenience fee if fees are not paid by the agreed upon deadline as listed in the Season 15 packet and weekly email. Athletes whose accounts are paid in full to receive the loyalty discount are not subject to this paragraph.

I understand and agree to the following:

1. The athlete will arrive on time and attend ALL practices, team events, and competitions.

2. I am expected to assume financial responsibility for the required items (shoes, additional uniform items, professional fees, tuition, etc.) I understand that cheer uniforms and shoes are not included in the tuition or professional fees and must be paid separately. Additional items may be required throughout the year due to sizing changes, loss, and wear-and-tear. I understand that if any items are lost or damaged, I am financially responsible for the replacement of those items.

3. I understand the team fees as listed in the all star packet and any questions I have regarding fees has been answered. 4. I understand that choreography, music, and logos are the intellectual property of Twister Sports. Unauthorized use for individual repurposing (apparel, mom shirts, athlete spirit gifts, etc.) is not legal without the written permission of Twister Sports.

ASW AGREEMENT

I understand if my team gets a bid to the All Star World Championship in Florida in April 2027, we will be expected to attend. Additional fees apply.

PAYMENT INFORMATION

Will you maintain enrollment through tryouts?*

For those who maintain enrollment up to tryouts, the Assessment Fee ($85) will be waived. This assessment fee is part of your annual professional fees. For those who do not maintain enrollment on an all star team, the fee will be charged upon submission of this form. Alternate tumbling and flexibility classes are offered on "off weeks" through the front desk.

We will not be officially enrolled at Tryouts.

Season 15 Elite Membership Options (Elite Teams Only)*

Elite athletes will have different membership options for Season 15. Please select the membership you would like for the entire season. Payments will begin May 1. Bronze Membership: $169/mo. Includes Elite Cheer Team, Team Tumbling Class, 6-week Summer SWAG Conditioning Session, Competition Bow, Practice Tank Top, and Weekly Open Gym. Silver Membership: $239/mo. (Early Bird pricing has ended) Includes everything in Bronze, plus 1 Semi-Private Lesson/month, 2 Open Tumbling Clinics/month, and Flyers Class. Gold Membership: $329/mo. Includes everything in Bronze, plus 2 Semi-Private Lessons/month, 2 Open Tumbling Clinics/month, Flyers Class, 1 Cat Haley Mastery Session, 1 Skills Camp/year, and a 3-Week Jumps Session. Athletes will select their package upon registration for Season 15. Packages may not be downgraded during the season, as many benefits are front-loaded at the beginning. Loyalty and pay-in-full discounts apply to the base tuition rate only.
